Membership Application

We are pleased to see that you would like to become a member of the Louisiana State Firemen's Association.
In order to qualify for individual membership you will need to complete these simple steps.

1. Verify that your Department is a member of LSFA. Your Department must be a member for you to qualify for individual membership. If your department is not yet a member of LSFA they can sign up now using our online form - Click here

2. Once your department is signed up you can then fill out the printable online form for individual membership - Click here

3. Mail the printed form along with your dues to the address listed on the bottom of the printed form. Each individual will need to print their own form.
